The current upcoming deadlines for each
program are listed on the
Childcare and
Education pages.
Students may apply for one or both programs but
the process is separate and funding for each is
considered separately. Some students may
be accepted for one program but waitlisted or
decline for the other. |

Shoe Voucher and Backpack Application |
| Instructions: |
Due to demand, the
short Shoe Voucher only applications can now
only be accepted from Public School Family
Support Workers (including Head Start) or case
managers working within domestic violence
shelter programs. If you are not a a
school social worker or working at a domestic
violence shelter and only want to request shoes,
please have the family contact their child's
school for a referral. Applications not made through
a school or other qualified referrer will be discarded without further contact. |

Emergency Family Assistance Application |
| Instructions: |
This for is
for Public School Family Support Workers, other
professional Social Workers or Public Health Nurses to get fresh copies of the
Emergency Family Services Application to provide written referrals
to the Seattle Milk Fund. Services vary
but can include food assistance, shoe vouchers,
baby supplies and household supplies. More
details are listed here. Please note that the
referrer's information is always confirmed before services are provided.
Applications not made through
a referring agency or school will be discarded without further contact. |

These forms
are for use by renewing Childcare and Education
Grant applicants. Do not submit these
forms unless you are already receiving a grant.
If you are submitting the renewal for Childcare
and Education, you must submit TWO copies of
your schedule and grades, one for each renewal
from. Please write "Education" on top of
the schedule and grades for the Education
Committee and "Childcare" on top of the schedule
and grades for the Childcare Committee.
